Service Connection is part of the Care Finder Network and is proudly delivered in partnership by healthAbility and Access Health & Community
A Service Connector can meet with you, where you like, and talk about your situation to understand what you may need. This could be anything from getting help around the home, support with shopping or having your groceries delivered, having a nurse visit, or arranging for someone to have a coffee or share an activity with.
This could include linking you with local community centres, aged care service or whatever you like. Service Connection is available to people living in the cities of Boroondara, Banyule, Nillumbik, Whitehorse and the lower areas of Murrundindi.
